(2) The reasons for the failure of the fasteners are: the glands, gland nuts, bolts, ears and other parts are broken; or they are loosened due to vibration; or damaged due to corrosion; or due to uneven force and excessive force. , The fastener is damaged by improper operation; or due to inadequate maintenance.
